How to create/bulk upload Form-based Risk Assessment?

1. In the 'Document Summary' page, click on 'Add Documents' icon and Select the 'Risk Assessment-Form' from the list and click 'Save' button.

2. Click on this Form icon to to view Risk Assessment.

Note: Before creating any Risk, define the Risk Matrix by clicking on 'Define Matrix' icon.

3. Choose from the list of Matrices, define their range and click 'Save' button.

4. Now Click on 'Add Risk Assessment' to add new Risk

5. Fill the mandatory fields like Reference, System/Sub-system, Failure modes, Probability, Severity, Detectability, fill other fields as required and Click 'Save'


  • The RPN is auto-calculated and Risk Priority is thus determined.

Note: For risks, we can create residual risk later as explained here.

  • The Risk is saved and Risk Id is auto generated.

6. Click on 'Bulk Upload' to upload multiple Risks together.


7. Click on 'Sample Download' to download the Excel based Risk template

8. Update the excel file and use 'Choose File' to upload the data

9. The records will be displayed on screen for confirmation. 

10. Click 'Import' to import the data. The records will be added with respective Risk Id.

11. Navigate to Document Summary. Click on Actions, then 'Table of Contents' to configure the PDF output.

12. Navigate to Document Summary and

  • Add References
  • Add Attachments
  • Click Preview to preview PDF output
  • Setup Workflow
  • Publish the document to take it for workflow approval
